I affirm that I, as well as all household members, have not been diagnosed with COVID-19 WITHIN THE PAST 30 DAYS. 3. I affirm that I, as well as all household members have been wearing masks outside of the home and taking proper precautions to avoid contamination. 4. I affirm that I, as well as all household members, have not traveled outside of the country, or to any city considered to be a "hot spot" for COVID-19 infections WITHIN THE PAST 15 DAYS. 5. I understand that If I, as well as all household members, are experiencing or have experienced any of the symptoms listed above WITHIN THE LAST 15 DAYS OF THE CONFIRMED APPOINTMENT DATE , I must contact my service provider directly to reschedule the appointment at least 30 DAYS from the current appointment date. 6. I understand that Tempest Salon cannot be held liable for any exposure to the COVID-19 virus caused by misinformation on this form or the health history provided by each client.

We always strive to reschedule appointments when something comes up; however, if your appointment is canceled or needs rescheduling within 36 hours of your scheduled date/time, I reserve the right to charge a 50% fee, unless I can fill the appointment last minute.
